What Is the Program?

The Healthy at Home Monitoring Program uses a Fitbit device and the MiHealth app to help your care team better understand how you are recovering at home.

The Fitbit collects information such as your activity level and other health signals. These measurements may help identify early signs that your recovery is not progressing as expected.

What You Will Be Asked To Do

If you choose to participate:

  • You will be given a Fitbit to wear for at least 30 days after discharge.

  • We will help you install the Fitbit and MiHealth apps on your smartphone.

  • Your health information will be securely shared with authorized members of the care team.

  • Your team may contact you with general recovery guidance if concerning changes are noticed.

There is no out-of-pocket cost for this program.

This program does not replace your doctor, emergency care, or routine medical follow-up.

If you are having a medical emergency, such as severe shortness of breath, chest pain, a fall, or another urgent problem, call 911.

Why Participate?

The first few weeks after leaving the hospital can be challenging. This program is designed to provide an added layer of support while you recover at home.

By participating, you may help your care team recognize recovery problems earlier and help us develop better tools for future patients.
